No! She’s properly introduced (ish) in The Wrong Doctors and Business Unusual. Both of them were intended to be Mel’s introduction but I believe The Wrong Doctors takes place first chronologically!
It’s a little like the trope “Monster Origin Story”.
There are monsters which have a “first appearance” in an episode like the The Daleks or The Tenth Planet, and then there are episodes where they go back to the original creation of the monster like Genesis of the Daleks, Spare Parts, etc.
I see it a bit like that, so maybe “Companion Origin Story” or something? Although that’s a bad name.

It would be odd if either of those stories were marked as her first appearance though wouldn’t it? Seeing as how they retroactively introduce an already established character from the main show
Yeah, I then think making a difference between a companions first appearance (our perspective) and this story type https://tardis.guide/story-type/new-companion-introduction/ (their perspective/Doctor’s perspective? Bit complicated with Charley then…) would work well.

I’ve just put in an edit for Stardust and Ashes as the runtime was wrong, I’ve noticed its been changed several times so not 100% sure why but its definitely what I’ve just put in
