So how come they don’t all know each others real names? Or if they do, why do they never use them?

Because they all don’t use each others dead names.

That’s because they were totally in this super-secret friend group and were using codenames to refer to each other, and those names stuck beyond their school days.

I’ve heard some fans mention how the “real names” are also fake names before The Doctor and The Master got their Doctorate and their Masters at the academy. (The same fans who make great art of this era).
I’m guessing it’s similar for The Rani…
